Urban Health Partnerships’ 8-Steps to Building a Successful Community Action Plan (CAP) approach is a practical, community-driven method that helps turn priorities into coordinated action. It brings together residents, Community Liaisons, organizations, and institutions to align efforts and implement solutions that improve health and well-being.
This approach is designed to lead to lasting change in the way communities function, not just short-term programs or one-time plans.

A core part of this approach is our Community Liaison (CL) Framework, which ensures that residents are not just engaged, but actively shaping decisions.
Community Liaisons are trusted local leaders who:
- Build relationships with residents and partners
- Gather real-time community insight
- Support outreach, engagement, and follow-through
- Help ensure plans reflect community priorities
This structure allows action plans to be built with communities, not for them, and strengthens accountability throughout implementation.
