The South Broward Community Health Hub (SBCHH)* has been focusing on work within the following Community Health Action Zones (CHAZ)** locations to root our relationships through the official signing of a partnership agreement:

  • Dania Beach
  • Hallandale Beach
  • Hollywood & West Park
  • Miramar
  • Seminole

We invite you to participate in our Debut Collaboration meeting on Thursday, October 26, from 11:00-1:00 pm to:

  • Learn about South Broward Community Health Hub
  • Hear from partners about local community activities and initiatives in South Broward
  • Work together to collaborate together on grants
  • Network and strengthen relationships with partners
  • Become an official partner of the South Broward Community Health Hub!

**Definition of CHAZ: A Community Health Action Zone, or CHAZ, is a small geographical region between 5 – 10 square miles where community members, partners, and other local stakeholders are already actively engaged in combating social determinants of health and building equitable food and health systems and opportunities in the area.

*About South Broward Community Health Hub

South Broward Community Health Hub (SBCHH) supports sustainable and equitable solutions to improve local health services and food access for vulnerable residents while honoring historical roots and cultural appropriateness. To do this, SBCHH collaborates with residents, neighborhood leaders, Community Liaisons (CLs), stakeholders, and representatives in South Broward to create partnerships and increase engagement. Our shared vision is to:

  • Coordinate community-driven and equitable activities, services, and opportunities related to food and health access
  • Leverage and strengthen local networks and communities to generate, supply, acquire, and sustain food, health, and other essential resources and opportunities
  • Establish inclusive and accessible connections for opportunities and resources, promoting physical, social, and economic well-being and success for individuals of all backgrounds and abilities

The South Broward Community Health Hub collaboration aims to engage and work with local stakeholders to create solutions for food and/or health insecure residents. The following funds at the Community Foundation of Broward have provided support for the South Broward Community Health Hub:
