Discover Our 2025 Impact: A Year of Resilience, Action, and Community-Led Progress

In 2025, Urban Health Partnerships remained grounded in what matters most: community. In a year marked by uncertainty and rapid change, we worked alongside residents, partners, and local leaders to respond to urgent needs while continuing to advance long-term progress across South Florida.

From navigating unexpected funding challenges to helping lead a regional response to food insecurity, this year demonstrated the power of collaboration in action. UHP launched the Community Food Finder app, supported efforts to strengthen food systems infrastructure, advanced transportation safety planning in Broward County, and introduced the 2025–2030 Age-Friendly Community Action Plan in Miami-Dade County.

Throughout it all, community voices remained at the center. Residents and local leaders worked alongside institutional partners to shape solutions, with Community Liaisons ensuring that lived experiences informed planning, programs, and decision-making.

2025 was not without its challenges, but it reinforced why this work matters. It was a year defined by resilience, shared responsibility, and meaningful progress toward healthier, more connected communities.
