Throughout the month of June 2021, Urban Health Partnerships, in partnership with the South Florida Geriatric Workforce Enhancement Program (SFGWEP), provided four (4) free learning opportunities for health care practitioners, with an emphasis on nurses, focused on the Age-Friendly Public Health System 4M Framework developed by the Institute for Health Care Improvement. The recordings and presentations are now available! Review Modules 1-4 below:

Series Objectives

By the end of the training, participants will be able to: ​

  1. Understand the need for Age-Friendly Health Systems (AFHS) ​
  2. Communicate the AFHS 4’M model​
  3. Identify your scope, role, and opportunities to practice the 4Ms in the healthcare setting

 

WATCH NOW: Module I: Introduction to Age-Friendly Health Systems

The Module I training was focused on introducing age-friendly health systems with Dr. Kevin O’Neil of ALG Senior, Dr. Naushira Pandya of the Department of Geriatrics at Nova Southeastern College of Osteopathic Medicine, and Isabel Rovira of Urban Health Partnerships. The speakers provided an overview of the 4M Framework, developed by the Institute of Health Care Improvement, and shared informative data and resources available to health care practitioners and nurses.

WATCH NOW: Module II: Deep Dives – What Matters Most and Mobility

Module II took a deep dive into What Matters Most and Mobility with presenters Lil Banchero of Luminius Anne Arundel Medical Center, Christine Waszynski of Hartford Hospital, and Isabel Rovira of Urban Health Partnerships (UHP). The Module II training was focused on providing specific examples of ways that health care systems can address “What Matters” & “Mobility” in age-friendly public systems. This training also highlighted what the 4 M’s means for healthcare practitioners.

WATCH NOW: Module III: Deep Dives – Mentation & Medication

Module III took a deep dive into Mentation and Medication with presenters Dr. Raymond Ownby of Nova Southeastern University, Dr. Todd James of UCSF Division of Geriatrics, and Isabel Rovira of Urban Health Partnerships. On the topic of mentation, participants learned from sources of evidence that cognitive decline may be preventable​, about the three modifiable lifestyle factors related to improved brain health​, and two strategies for encouraging behavior change for brain health. On the topic of medication, participants learned about resource for identifying Potentially Inappropriate Medications, learned about risks in four specific clinical scenarios for older adults​, learned about evidence which alters management for four common practices​, and how to formulate improved care plans engaging interprofessional resources.

WATCH NOW: Module IV: Putting It All Together

Module IV was the final training module of the Age-Friendly Public Health Systems 4M Training for Healthcare Practitioners series. Module IV introduced participants to the Trust for America’s Health, a background on age-friendly public health systems, the development of the Florida Department of Health’s Aging in Florida Profiles, and concluded with a demonstration of Aging in Florida Profiles.
