Age-Friendly Health Systems | 4M Training for Healthcare Practitioners

Urban Health Partnerships (UHP), in partnership with the South Florida Geriatric Workforce Enhancement Program (SFGWEP), hosted an Age-Friendly Health Systems 4M Training Series in Spring 2022.

The overall focus and theme of the 4-part session series was, “Putting It Into Practice: Case-Based Approaches to Age-Friendly Health Systems”. This virtual training series focused on the Age-Friendly Health Systems 4M Framework, an initiative of The John A. Hartford Foundation and the Institute for Healthcare Improvement in partnership with the American Hospital Association and the Catholic Health Association of the United States. Renowned speakers from across the country provided insight into the implementation of Age-Friendly Health Systems.

Training Length: Each module was approximately 90 minutes.

Each training session took place at 12:00 p.m. Eastern Time

  • Wednesday, June 1, 2022: The Patient Side of Age-Friendly Health Systems
  • Wednesday, June 15, 2022: Challenges, Barriers, and Solutions to Implementing AFHS
  • Wednesday, June 29, 2022: Panel Discussion: The Importance of Integrating Diversity, Equity, and Inclusion and Workforce Resilience

Training Cost: This training was provided at no cost to participants.
