About Accelerate Change Together

The City of Miami LGBTQIA+ SDoH Accelerator Plan (also known as Accelerate Change Together) is an initiative to address health inequity in the City of Miami LGBTQIA+ community. The initiative is funded by the Centers for Disease Control and Prevention (CDC), led by The City of Miami, and implemented with support from Urban Health Partnerships (UHP) and community partners. The goal is to strengthen the community’s capacity to collaboratively plan and collectively carry out strategies to close the Social Determinants of Health equity gap, thereby improving the health of the LGBTQIA+ community.
